I hereby WITHDRAW MY CONSENT granted to this Mutual Society to carry out electronic communication for processing, as long as the contractual relationship lasts.

I have been learnt more about the possibility of practicing my access rights, rectification, suppression, limitation, opposition and/or portability addressing to the Delegate of Data protection (DPD) of Mutua Universal through email to protecciondatos@mutuauniversal.net or by mail to the following address: Av. Tibidabo, 17-19, (08022) Barcelona.

I have also been informed on my right to submit a claim to the Spanish Data Protection Agency (AEPD), through its website: www.aepd.es
