The medical care form is a document the company can use to request medical care for the injured worker at our help centres.
The company is responsible for informing us of the workplace accident and giving the medical care form to the injured worker, who must present the form including their personal details, those of the company and details of when and how the accident happened.
This form can be obtained in electronic format and downloaded, so it can be filled in, or generated automatically by accessing the "medical care form" tool in our private area.
It should be remembered that this form is not in any way a substitute for the accident report.
After the form has been presented, the company must also fill in the accident report or include the case in the list of cases without sick leave, unless our medical services decide that it is not work-related.
